Chronic Kidney Disease Case Management

Started under the auspices of our Population Health Department, we have a team of dedicated nurses and case managers who care for patients with chronic kidney disease, providing education, care coordination and help reduce hospitalizations and ER visits. Our “PD first” policy that has gained traction system wide and we are proud to partner with our colleagues in Interventional Nephrology and Surgery to have timely PD catheter placements.

Patient Education

Dialysis Access Services

Interventional Nephrology

Interventional nephrology is a subspecialty of nephrology that focuses on performing procedures related to kidney diseases and dialysis access. Interventional nephrologists use ultrasound, catheters, and endovascular techniques to diagnose and treat various dialysis access issues. Our interventional nephrologists can provide tunneled dialysis and infusion catheter placements, dialysis fistula and graft angiograms and declots, as well as peritoneal dialysis catheter troubleshooting.

Dialysis Access Clinic

A dialysis access surgeon is a surgeon who specializes in creating and maintaining vascular access for hemodialysis patients as well as peritoneal catheters for peritoneal dialysis patients. Our surgeons work closely with our nephrologists to ensure excellent individualized care is provided for our patients who are either preparing or presently on dialysis.

Glomerular Disease Clinic

At the Glomerular Diseases Clinic, we offer cutting edge, state of the art management options for patients with glomerular disease such as vasculitis, lupus nephritis, primary glomerular disease, to name a few. Outstanding renal pathology services are run by our in-house renowned renal pathologists, providing rapid turn around for renal biopsy reports. A planned expansion of this clinic includes liason with rheumatology to offer multidisciplinary services for patients with autoimmune disease.

Transplant Clinic

Our kidney transplant program provides comprehensive care for patients with end-stage renal disease who are awaiting or have received a kidney transplant. Our transplant program is highly active, innovative, and provides excellent outcomes for both living and deceased kidney donation.
