Family

We know that your health care choices mark the beginning of a new and very important relationship.

When looking for a personal physician, you want a doctor who not only sees you when you need to be seen, but who also sees you as a unique individual.

And as an academic organization thats constantly studying how to improve the practice of medicine, we know how important it is to work with you to understand not just your symptoms, but also your perspective, concerns, ideas and expectations.

Learning about your family life, career and interests creates valuable context about your health. And involving you to reach an agreement about managing your health means that you and your physician can then work together toward your goals.

These are among the reasons that your UC Davis primary care team is structured as a patient-centered medical home. This model of care is designed to help you live a healthier life by taking a team approach to supporting your personal needs. This allows your personal doctor to spend more time listening to you, and getting to the heart of your health goals and challenges.

Were proud that UC Davis Medical Group primary care clinics have earned the prestigious Patient-Centered Medical Home designation from the nonprofit National Committee for Quality Assurance (NCQA). Medical homes emphasize shared decisions to help patients to make informed choices and receive better results. They also stress strong communication and coordination of care among all of a patient’s health care providers, and offer patients strong access through expanded hours and technologies such as online communication.

Our focus on patient-centered care is grounded in our own longstanding research about the importance of human interaction in medicine. For example, UC Davis studies have found that physicians who have more personalized discussions with patients, and encourage them to take a more active role in their own care, can help lower medical costs and reduce the need for some services.

Were also very proud that UC Davis primary care comes highly recommended by our patients!
