My goal as a pharmacist is to provide patient-specific clinical monitoring and pharmacologic education while serving as a patient advocate. Through review of medications and social factors, I anticipate potential barriers as patients transition throughout the care continuum. I am also dedicated to supporting the education of pharmacy learners through didactic and experiential education.
Transitions of Care
Dr. Yvette Mitchell is a Transitions of Care pharmacist at UC Davis Health where she serves as a preceptor for students and residents. She completed her PGY-1 and PGY-2 in Ambulatory Care at UC Davis Health with a focus in academia during her second year of residency. She currently serves as an Assistant Clinical Professor at the University of California, San Francisco School of Pharmacy and provides support to the Greater Sacramento Area Experiential Education site. Dr. Hellier serves on various committees dedicated to educational development and co-coordinates the Scholarship of Teaching and Learning Program.
