UC Davis Health offers referring physicians four different ways to refer patients to us. Referrals are processed within 48 hours once all necessary information has been received by us.

Electronic Referral Intake Form
Easiest for small physician clinics; no set-up required.
- Submit your referral using our secure online form
- Upload clinical notes, labs, imaging documents
- Include proof of insurance and authorization details
Care Everywhere Referrals Management (CERM)
Recommended for clinics and health systems using Epic.
- Send and track referrals in Epic
- Send messages to UC Davis Health referral teams
- Receive automatic patient appointment and care updates
Direct Messaging and 360X Referrals
Best for organizations using Direct Secure Messaging.
- Enhanced security of protected health information (PHI)

PhysicianConnect
A secure online portal that uses UC Davis Health’s Epic system.
