Frequently Asked Questions - Release of Images
Patients may electronically request copies of their radiology images via MyUCDavisHealth. Images will be mailed on a CD or sent via secured email to the patient directly. Email or mailing address is required. Walk-in Service is not available.

We process requests in the order that it is received. Please allow us up to 15 days to process your request. We encourage you to provide your email address so the radiology images can be sent via secured email to you directly. After a request is processed on a CD, it is mailed out by regular mail through USPS. Please allow 7-10 business days to received it.
To check status of your request after 15 days, send an email to ucdhsimagerequest@ucdavis.edu. Please be sure to include name and date of birth of the patient so that we may better help with your request.
No, UC Davis Health does not have a walk-in service.

In general it can take up to 15 days to complete your request. To check status of your request after 15 days or if you are experiencing technical issues retrieving records that you received, you may email ucdhsimagerequest@ucdavis.edu. Please be sure to include name and date of birth of the patient so that we may better help with your request.

Patient's may also electronically request and receive copies of their medical records via MyUCDavisHealth
If your external physician is a member of a health system that already uses the EPIC electronic medical record system, they can view and pull your UC Davis Health medical records electronically through the EPIC CareEverywhere module. No enrollment is required for this feature. Your external provider can use their EPIC system to retrieve the medical records.
If your doctor's office does not use the EPIC electronic medical record system, they may fax a STAT request on letterhead to 916-734-2126 or email to hs-roi@ucdavis.edu. Please make sure the urgency is indicated on the request.
Patients requesting copies of their own records will take no longer than 15 days to process. All other requests will take no longer than 30 days to process.
You may email hs-roi@ucdavis.edu for medical records to check the status or if you are experiencing technical issues retrieving records that you received. Please be sure to include name and date of birth of the patient so that we may better help with your request.
For patients there may be a charge of 25 cents for each page plus the cost for shipping and handling. There is no cost for receiving records via MyUCDavisHealth.

No, due to the confidentiality of the records, we are not able to fax records to you.

Download the Request to Amend form or via MyUCDavisHealth. Email completed amendment to hs-roi@ucdavis.edu. We will review and respond to your request within 30 days.
Yes, for paper charts only. Once your authorization form is submitted we will contact you to schedule an appointment. Electronic Medical Records are available via MyUCDavisHealth.
No. A separate form must be completed and signed for each family member. For those family members who are 18 years of age or older, legal documentation must be provided when signing on behalf of the patient.
Children born at UC Davis Health in Sacramento would obtain a birth certificate from the Sacramento County’s Clerk/Recorder. While UC Davis Health maintains medical records, we do not maintain birth certificates. All birth certificates are maintained by county government offices in the county where the child was born.
