Release of Images

Health Information Management Release of Images Department has virtualized all Release of Information Services.

Business hours are Monday to Friday, 8 a.m. to 4 p.m., excluding weekends/holidays. You may also visit our frequently asked questions page for more information.

• Patient's may electronically request copies of their Radiology images via MyUCDavisHealth (MyChart)
• Authorization for Release of Information:  Electronic Submission or Printable PDF
• Please submit your completed authorization to:

• Patient: Images will be mailed on a CD or sent via secured email (email or mailing address is required).
• Provider: Images will be mailed on a CD or sent via an electronic image sharing platform.

UC Davis Health strongly advises to utilize an electronic sharing platform.

• In general it can take up to 15 days to complete your request. To check status of your request after 15 days, send an email to hs-ucdhsimagerequest@ucdavis.edu. Please be sure to include name and date of birth of the patient so that we may better help with your request.

• If you would like to add images done outside of UC Davis Health to your legal medical record or send images for comparison to a Radiology exam, please contact your provider or the Radiology Department below:
