A written authorization form must be submitted to the Health Information Management. The form can be mailed, faxed, or scanned and sent via email. Please use the contact information below.

Patient's may also electronically request and receive copies of their medical records via MyUCDavisHealth
Children born at UC Davis Health in Sacramento would obtain a birth certificate from the Sacramento County’s Clerk/Recorder. While UC Davis Health maintains medical records, we do not maintain birth certificates. All birth certificates are maintained by county government offices in the county where the child was born.
Patients requesting copies of their own records will take no longer than 15 days to process. All other requests will take no longer than 30 days to process.
For patients there may be a charge of .25 cents for each page plus the cost for shipping and handling. There is no cost for receiving records via MyUCDavisHealth.

If you or your external physician have questions about medical records, please contact UC Davis Health's Health Information Management Department at 916-734-5205 (Telephone hours are Monday to Friday, 8 a.m. to 4 p.m., excluding holidays). Due to the high volume of calls, email or fax method is highly encouraged. Closed for Walk-In Service.

Patient's may electronically request copies of their radiology images via MyUCDavisHealth (MyChart). Images will be mailed on a CD or sent via secured email to the patient directly. Email or mailing address is required. Walk-in Service is not available.

We process requests in the order that it is received. Please allow us up to 15 days to process your request. We encourage you to provide your email address so the Radiology images can be sent via secured email to you directly. After a request is processed on a CD, it is mailed out by regular mail through USPS. Please allow 7-10 business days to received it.
To check status of your request after 15 days, send an email to hs-ucdhsimagerequest@ucdavis.edu. Please be sure to include name and date of birth of the patient so that we may better help with your request.

Yes, for paper charts only. Once your authorization form is submitted we will contact you to schedule an appointment. Electronic Medical Records are available via MyUCDavisHealth.
No. A separate form must be completed and signed for each family member. For those family members who are 18 years of age or older, legal documentation must be provided when signing on behalf of the patient.
Yes. You can view your medical records and even contact your Physician using our MyUCDavisHealth service. Sign-Up for MyUCDavisHealth!
