Children born at UCDMC in Sacramento would obtain a birth certificate from the Sacramento County’s Clerk/Recorder. While UCDMC maintains medical records, we do not maintain birth certificates. All birth certificates are maintained by county government offices in the county where the child was born.
Patients requesting copies of their own records will take no longer than 15 days to process. All other requests will take no longer than 30 days to process.
For patients there may be a charge of .25 cents for each page plus the cost for shipping and handling.

We need to receive a request on letterhead from your physician which indicates which records are needed. The request needs to be faxed to 916-734-2126.
The front desk is open Monday through Friday, 8:00AM - 5:00P.M. Closed most major holidays.
No, due to the confidentiality of the records, we are not able to fax records to you.

Yes, we can schedule an in person review for you. Once your authorization form is submitted we will contact you to schedule an appointment.
No. A separate form must be completed and signed for each family member. For those family members who are 18 years of age or older, legal documentation must be provided when signing on behalf of the patient.
