  • Health Sciences Compensation Plan Faculty

    The following leave packets apply to faculty appointed in a Health Sciences Compensation Plan title.

    • Pregnancy Leave (PDF)
      An employee disabled by pregnancy, childbirth, recovery or related medical conditions.
    • Parental Leave/Childrearing (PDF)
      An employee’s leave to bond with their new child entering the family through birth (non-Childbearing), surrogacy, adoption, or foster care placement, and to attend to related matters.
    • Employee’s Serious Health Condition (PDF)
      An employee disabled due to an illness, injury, impairment, or physical or mental condition that involves inpatient care or continuing treatment by a health care provider.
    • Family Serious Health Condition (PDF)
      An employee’s eligible family member disabled due to an illness, injury, impairment, or physical or mental condition that involves inpatient care or continuing treatment by a health care provider.

Additional Leave Information

  • FMLA Tracking Sheet (EXCEL)
    Enter basic employee information for formula assistance tracking FMLA usage. Particularly helpful for intermittent FMLA, but also available for tracking paid leave and other designations (PDL, CFRA).
  • Sabbatical Credit Calculation Worksheet (EXCEL)
    A spreadsheet with formulas built in to double check sabbatical credit accruals for faculty exploring taking a sabbatical.
  • MyInfoVault (MIV)
    For leave approvals: Log into MIV with Kerberos credentials. In the top, left corner of the Home Page, you’ll see a section for Forms. Select “Manage Forms” to start drafting a new leave request.
    All leave types (besides faculty vacation) must be filed in MIV.
