Required Onboarding Documents

  • Background Check (PDF)
    This takes approximately one week to process. The background check form must be signed by the applicant. Rehires: Resubmit if more than six months have passed since the last background check was completed. Resubmit with a change in position or department.
  • I-9 Form: Processed through Tracker
    Rehires: As stated in the I-9 directions, “When rehiring an employee within 3 years of the date Form I-9 was originally completed, employers have the option to complete a new Form I-9 or complete Section 3.
    Not required if no break in service:
    • Break in service: 1 day or more
    • Not considered a break if transferring from campus, and no break in service
  • UC State Oath of Allegiance, Patent Policy, and Patent Acknowledgement (PDF)
    Oath is not required for International / Non-U.S. Citizen employees; however, the bottom half of the Patent Policy and Patent Acknowledgement is still required. Rehires: Complete if more than one year from the last date oath/patent signed.
    Not required if no break in service:
    • Break in service: 1 day or more
    • Not considered a break if transferring from campus, and no break in service
  • Personal Data Form (PDF)
    Required for new hires and if transferring from campus with no previous PSoft appointment.
  • Confidentiality Agreement Form (Word) 
    DO NOT SUBMIT - retain document in the department file.
  • Elder Abuse Notification Form (PDF)
  • Child Abuse Notification Form (PDF)
  • Compensation Plan Statement (PDF)
  • Disclosure Clearance (for recruitments open after January 1, 2025)

Recall Information

Recall to active service requires pre-approval. All recall requests may be initiated by the department using the Academic Affairs Forms Online system.
