Download donor application (PDF)Complete the donor application document and return it to the Body Donation Program. You will be notified if you've been accepted as a registered donor and an identification card will be issued. This card will have your donor number along with a phone number to call at the time of death. Your physician and/or care facility should be notified of your wishes and informed of the contact procedure.
