Specimen Labeling

Specimens must be labeled with at least 2 patient identifiers.

Test Requisition Forms

For all non-interfaced clients, a paper requisition form is required with submission of each sample.

Transport or Shipping of Specimens

It is highly recommended that a packing slip be included with each shipment. Please reach out to laboratory staff if you would like to request a faxed confirmation of receipt of specimens.

Hospital Couriers – in the event the building receiving area is closed, please call 530-752-1757 upon arrival and laboratory staff will receive the specimen directly.
