    Join us as we visit two Napa wineries on Saturday afternoon after the last session of the symposium. Registration ($20) includes a box lunch and tasting at two wineries. Transportation to and from the wineries has been generously provided by Drs. Chris Serdahl and Clarissa Tendero, and Dr. Mike Schermer and Shelley Schermer. (Limited to first 62 registrants; must be a registered participant or guest of a registered participant)
  • 2 Day Rate for Saturday & Sunday attendance

ENROLLMENT

Please register early – space is limited.

  • Tuition is $549 for physicians and $349 for all others. The fees will increase after April 6 and May 10.
  • Tuition includes continental breakfasts, refreshment breaks, electronic syllabus, and an online certificate of attendance via the American Academy of Ophthalmology. Tuition may be paid by check, American Express, Discover, MasterCard or VISA. Cash is not accepted. The UC Davis Eye Center reserves the right to cancel this program. In such a case, a full refund will be given.
  • Registration forms received without payments will not be processed until payment is received, and payment must follow within 10 days. Early discount rates are honored if payment is postmarked by the date noted on the enrollment form. After that date, the late fee is applied.

Cancellation and Refund Policy:

A refund of tuition, less $100 administrative fee, will be allowed if requested in writing by April 13. No refunds will be provided after this date.
