Clinical Laboratory Science Training Program: Application Procedure

The following needs to be completed online:

• Online Application (link) which includes an application form, course history form, and a work history form.
• A personal statement, directions within the application portal.
• Three recommendation letters (an online link will be provided to each listed recommender via the application website).  Acceptable recommenders are supervisors and instructors in any combination.  Recent recommendation letters are preferred.
• Official transcripts are NOT required with initial application.  Applicants selected for interview will be advised on when to send in official transcripts to the program office.  All CLS training acceptances are contingent upon the self-reported GPA being confirmed via official transcripts.
