Clinical Pathology Testing Inquiries
- Fill out Client Account Form (PDF), Secure Fax Form (PDF) and Testing Utilization Form (PDF).
- Submit completed forms to: ucdavishealthlaboutreach@ucdavis.edu.
- A Lab Outreach Representative will contact you in 2-3 business days.
- Upon receipt of the above documents, a Lab Outreach Representative will provide pricing and a proposed scope of work for your review. If the terms and pricing are acceptable, UC Davis Health will execute a contract for services. Your representative will work with you to begin services in good faith while the contract is being ratified.
