Your primary care doctor or specialist may refer you for an integrative medicine consultation so that one of our experts can provide personalized recommendations to address your health concerns. To make the most of your visit, please download and complete the integrative medicine new patient packet and bring it with you to your visit.
This packet will give your doctor more in-depth information about you, your life, and your personal goals. You may meet with your doctor just once or several times, depending on your unique circumstances.
