New Patient Appointments

Request a referral from your physician to our Department of Orthopaedic Surgery. If you have a primary care doctor, talk to them about referring you to a specialist. 

  • Most UC Davis Health medical specialists require a referral from either a primary care physician or another specialist, either in the community or at UC Davis.

Returning Patient Appointments

If you are an existing UC Davis Health patient, you can request an appointment with your primary care physician using MyUCDavisHealth, a free, secure and confidential online portal for communicating with your doctor’s office.

Request a referral

Organizing your information and questions before you see your Orthopeadic team of providers can be very helpful both to you and to your care team. Here are some tips to help you make the most of your first appointment.

Bring notes about your personal health history, including the dates, for the following:

  • Your past and present health problems
  • Any surgeries or procedures
  • Tests and the results, if known
  • Any Allergies
  • Vaccines you have received
  • Any supplements and vitamins you are taking including; over-the-counter medications
  • Health information about your parents, grandparents and siblings, if known
  • Also, write down any of the following:
  • Questions you have for your healthcare team; underline or highlight a few of the most important questions
  • Any new symptoms

Other suggestions:

  • Ask a family member or friend to come with you to the visit, to listen and take notes while you are talking with your healthcare team
  • Bring your insurance card and identification
  • Bring a copy of your advance directive or durable power of attorney for health decisions

We accept a wide variety of insurance plans, but authorization requirements vary by network. Our team works with referring providers and insurance plans to obtain necessary approvals. You may be contacted if additional information is needed

  • Verify Network Participation: Confirm that UC Davis Health is an in-network provider for your specific insurance plan. If we aren't part of your insurance network of primary care doctors and specialists, the type of insurance you have may result in additional fees to see an out-of-network provider.
