Thank you for considering living kidney donation. Please read through our "how to guide" before submitting your Living Kidney Donor Questionnaire.

When submitting your questionnaire you will be redirected to our EMR system:

If you have questions after reading through the guide, or require additional assistance submitting your form, please call the living donor transplant office at (916) 734-2307. Once the online form is completed, it is automatically sent to the living donation team for review.  After review, a member of the team will contact the potential donor.
