Getting Started

Thank you for considering living kidney donation. Before you get started, please note that all potential applicants must have a UC Davis MyChart account to complete the Living Kidney Donor Questionnaire. You may need to create one or use an existing account if you have previously used UC Davis MyChart. We also ask that you please read through our "how to guide" before submitting your Living Kidney Donor Questionnaire.

If you have questions after reading through the guide, or require additional assistance submitting your form, please call the living donor transplant office at (916) 734-2307. Once the online form is completed, it is automatically sent to the living donation team for review.  After review, a member of the team will contact the potential donor.
