CIRCLE Clinic is an integrated pediatric primary care clinic for children and families currently involved with the child welfare system, in addition to children and families who are experiencing sequelae of childhood trauma and in need of assistance navigating healthcare. This clinic was founded to meet the unique and complex health needs of children in foster care, with the goal of closing care gaps and preventing fragmentation of care. CIRCLE Clinic is a multidisciplinary primary care clinic offering full-scope medical care, as well as mental health support and family resource navigation at every visit.

CIRCLE Clinic operates as a partnership between the Sacramento County Health Center and UC Davis Health. It is located at the Sacramento County Health Center on Broadway in Sacramento, and staffed by both Sacramento County employees and UC Davis Health providers. Children with developmental concerns are seen by MIND Institute Developmental and Behavioral Pediatrics specialists who are in clinic every week to provide assessment and on-going care. To help enhance caregiver-child relationships, all children are offered PC-CARE, a brief parenting intervention developed at the UC Davis CAARE Center; PC-CARE is offered to both resource caregiver and biological parents.

    Collaboration

    We recognize that a child is part of a family system, and we work collaboratively with all the members of the child’s ‘team.’ We practice an integrated and collaborative care model in which mental health, developmental and behavioral pediatric specialists, family navigators, care coordinators and general pediatricians work together to provide comprehensive patient-centered approach to medical care.
