• The UC Davis Department of Neurology accepts referrals from referring physicians only. If you are a referring physician who would like to refer a patient, please call the Physician Referral Center at (800) 482-3284 (option 2).

  If you are a returning patient and need to schedule an appointment, please call (916) 734-3588, Monday – Friday, 8 a.m. to 5 p.m.

• All referrals, refill requests, and patient requests should be faxed to 916-703-7911.
