Example #1:

“I have been working with Katy in the heart failure clinic since November 2016. In the very short time I've been working here, I've have been eye witness to the kind and compassionate care our patients receive from Katy everyday. Time and time again I have seen her go out of her way to help our patients in any capacity she can. One instance that comes to mind is a call we received from one of our patients in the hospital. He was highly distressed because he had broken his glasses and couldn't see. This patient was a particularly difficult patient. Nonetheless, Katy walked over to the hospital and arranged to try and have his glasses fixed. No matter what a patient requests, clinical or not, Katy always tries to help get them what they need. Whether it's a ride to an appointment, something to eat because the patient has been sitting in the clinic all day, or just a kind ear to listen to their complaints, Katy is always willing to help. She works with the most difficult of personalities and does it respectfully and patiently. She will often call patients just to check up on them and see if they are ok. Her kind spirit is matched by her strong clinical experience as a nurse. She is always willing to answer questions, whether it is from staff or a patient, and if she doesn't know the answer, she will find it for you. She has been an invaluable resource to me, as well as all her patients. While she is very involved in creating change within UCD through her service on professional governance councils, and is wonderful at promoting teamwork in the work place with her excellent communication skills, Katy's greatest strength is her big heart and her dedication to her patients. The heart failure clinic and all of UCDavis is lucky to have her.”

 

Example #2:

 “I would like to nominate Michelle Foster RN in the Pediatric ICU for this award. I have worked with Michelle for, I would estimate, over 15 years. She has been someone who always sets a high standard of care for patients and their families, but I am nominating her because she consistently goes above and beyond for so many families. In the PICU, we often have families whose children are in our care for extended periods of time. Sometimes we become attached to not only the children, but their families as well. I think that may people can take care of an ICU patient, but it takes someone truly special to see beyond the medical needs and give of themselves from their heart and even when not at work. I remember a young boy who suffered a TBI and who left the hospital quite impaired. Michelle went to their house on her own time on several occasions to provide respite care. For another baby who had been on and off of ECLS and had multiple heart surgeries in just a few months after birth, Michelle knew that the family had some financial hardships due to not being able to work while their baby was hospitalized. She bought them a bouncy seat for their baby and would buy them dinner on her way to work sometimes. She frequently brings in little homemade items like a mobile for a special baby she's taking care of and makes things like a sling out of bandnetting for the kid's stuffed animals, or a construction paper sign of encouragement- things that help make the hospital room a home. And not to mention, Michelle often comes in extra to help her peers during short staffing and is always willing to stay a little late if she needs to help a new nurse feel comfortable with her assignment or spend a little extra time giving report when need be. Michelle is a leader, preceptor, mentor, and educator to her co-workers as well, always with a positive attitude.”
