Residency Program - Case of the Month
February 2013 - Presented by Adam Stelling, M.D.
Clinical history:
The patient is a newborn female infant born at 37 weeks gestation with a left –sided multicystic lung mass discovered at 25 weeks on ultrasound. The mass occupied most of the left upper lobe and shifted the mediastinum to the right. There was a connection to the main bronchial tree and the cysts ranged from a few millimeters to 1.5 cm in greatest diameter. No other anomalies were noted on imaging. Two days after birth she underwent a left upper lobectomy due to acute respiratory decompensation. Intraoperatively the lesion was confirmed to be restricted to the left upper lobe.
Gross:
Received was a 22 gram, 8.1 x 5.5 x 2.3 cm lobe of lung. The surface appeared vaguely lobulated. Sectioning revealed multiple cysts ranging from 0.1 to 1.1 cm. Scant pale tan lung parenchyma was identified at one end of the specimen.
