UC Davis Health launches at-home care program transforming post-angioplasty care
New remote program aims to reduce hospital admissions for percutaneous coronary intervention patients
UC Davis Health announces the launch of a novel digital care program for patients who have undergone a Percutaneous Coronary Intervention (PCI). The program leverages the UC-NOW text-messaging platform to provide post-discharge care and Clinii to asynchronously monitor patients’ vitals.
PCI (previously called angioplasty with stent) is a minimally invasive procedure that opens blocked coronary arteries, often by ballooning a narrow segment and inserting a stent to improve blood supply. The time between the procedure and a follow-up office visit could be several weeks. This is a vulnerable period for complications.
“The first 30 days after PCI is the most crucial time. This program is a groundbreaking digital initiative that redefines post-angioplasty care for patients,” said Surabhi Atreja, associate professor of cardiology who is helping to oversee the program. “The heart procedure is just the beginning of a patient's healthy heart journey. By using cutting-edge technology that automates monitoring and education during the critical post-procedure period, we aim to ensure a seamless recovery journey, reduce heart risk factors, and enhance patient well-being.”
Patients enrolled in the program will utilize CliniiMobile Technology. The patients are provided a weight scale, blood pressure kit, and 4g hub that transmits data directly to the clinical team. As patients take their blood pressure or step onto the weight scale, their data will be sent to a dashboard accessed by UC Davis Health clinicians. These providers can then reach out to provide care and guidance when there are changes in vitals.

The UC Davis Health care team will monitor the data of 200 patients daily and take preventive measures to improve health outcomes and potentially help reduce hospital readmissions.
