Clinical preceptorship experiences

This is a general list of required documents for a preceptorship experience at UC Davis Health. All preceptor candidates must submit the following documents:

1. Graduate/undergraduate application
2. Preceptor invitation or email from department manager
3. Course objectives with course #
4. List of competencies/skills/capabilities if applicable
5. Copy of student or school’s liability insurance certificate
6. Approved Short Term Agreement between the sponsoring school program and UC Davis Health (allow 2 months to establish new contract) if no long-term training agreement exists.

Non-employees must also comply with the following requirements:

1. Completion of Attestation Letter on behalf of student
2. Completed eHR Access Agreement
3. Copy of Government Issued Picture ID
4. Current CPR card
5. Completion for UC Davis Health’s Mandatory Annual Training
6. Completion of Privacy and Security with accompanied HIPAA Confidentiality Form
