Anthem Grievance Form
UC Davis Health respects the patient’s right to express dissatisfaction regarding quality of care/services and to appeal any denied claim/service. Attached you will find the applicable appeal and grievance form for Anthem members. If you have a grievance against your health plan, you should first telephone your health plan, in accordance with their appeals and grievances process.

Our friendly front desk staff at any of our many clinics would be happy to assist you with printing the Appeals and Grievance form, upon request. The process for submitting the appeals and grievance form is as follows:
- Print the Anthem Appeals and Grievances Form (PDF).
- Populate the form with the appropriate information
- You may call the toll-free phone number on your member ID card to ask customer service to fill out the form for you.
- Attach any information you have to support the request.
- Send the form and any supporting information to:
