The department of Patient Relations is here to serve as a liaison between our patients, families, and their healthcare providers. We serve as the office of record for complaints and grievances for UC Davis Health. We first recommend that you discuss your concerns with the department leadership. If your concerns remain unresolved, our skillful and professional Patient Relations Analysts are available by phone, via email, or by submitting the online feedback form found below. Our goal is to partner with you to address any concerns that may arise.

Anthem Grievance Form

UC Davis Health respects the patient’s right to express dissatisfaction regarding quality of care/services and to appeal any denied claim/service. Attached you will find the applicable appeal and grievance form for Anthem members. If you have a grievance against your health plan, you should first telephone your health plan, in accordance with their appeals and grievances process.

Anthem: 1-800-365-0609 or at the TDD line 1-866-333-4823 for the hearing and speech impaired.

Our friendly front desk staff at any of our many clinics would be happy to assist you with printing the Appeals and Grievance form, upon request. The process for submitting the appeals and grievance form is as follows:

  1. Print the Anthem Appeals and Grievances Form (PDF).
  2. Populate the form with the appropriate information
    • You may call the toll-free phone number on your member ID card to ask customer service to fill out the form for you.
  3. Attach any information you have to support the request.
  4. Send the form and any supporting information to:

The California Department of Managed Health Care is responsible for regulating health care service plans. You may wish to contact the California Department of Managed Health Care. You may follow the California DMHC link to submit an online complaint or call the California Department of Managed Health care Help Center at 1-888-466-2219.
