Bio:
Dr. Wong is a board-certified ambulatory care pharmacist specializing in Transitions of Care at UCD Health. Her work involves medication reconciliation, patient education, clinical assessment, and medication coordination with healthcare facilities. She is committed to ensuring that patients’ medication regimens are safe and effective as patients transition in and out of the hospital setting. In addition, she collaborates with providers to optimize medication therapy, maximize adherence and ensure affordability based on each patient’s individualized needs and goals of care. She is involved in several interprofessional committees and works closely with physicians, nurses, and case managers to advocate for safe medication use and practices.
