The UC Davis Anticoagulation Clinic is designed to provide quality clinical and educational services for patients who are at risk for blood clots and taking medications such as warfarin, direct oral anticoagulants (apixaban, rivaroxaban, dabigatran), and injectable anticoagulants such as fondaparinux and enoxaparin.
We have an interdisciplinary team consisting of pharmacists, physicians and pharmacy technicians specially trained in anticoagulation therapy. We collaborate with other health care professionals to ensure our patients receive safe and effective care while being on anticoagulation therapy. Our team is responsible for ordering and monitoring labs and ensuring you know how much of your anticoagulant medication needs to be taken each day. Our pharmacy technicians can also assist with reviewing cost options for your anticoagulant medication.

The UC Davis Anticoagulation Clinic continues to provide the highest level of care and achieve the best possible outcomes for our patients on antithrombotic medications. Through access to evidence-based guidelines and constantly researched and updated best practices, tools and information, our healthcare professionals are improving outcomes through active participation in our patients' care.
Patients are established with the Anticoagulation Clinic through referrals from primary care providers or specialty providers at UC Davis Health. Our staff continuously collaborate with providers regarding patients' care. All patient visits are conveniently conducted via telephone. Once care is established, our team will support all coordination of care needs for warfarin. Our pharmacy team follows a detailed workflow model and provides clinical care based on the most up-to-date guidelines.
