Bio:
Dr. Sarah A. Bajorek is board certified in ambulatory care. She supervises the Population Health Transitions of pharmacists and pharmacy technicians. Dr. Bajorek graduated from UC San Diego (UCSD) Skaggs School of Pharmacy and Pharmaceutical Sciences, where she helped ensure patients could get access to their medications under her role as Pharmacy Clinic Manager for the UCSD Student-Run Free Clinic. She went on to complete a PGY1 Pharmacy Practice Residency at UC Davis Health and a PGY2 Transitions of Care Residency at UCSD. After completing her PGY2, she worked a year in Home Infusion Pharmacy to get an understanding of another aspect of care coordination.

Dr. Bajorek returned to UC Davis Health in 2014 to work as a Transitions of Care pharmacist, ensuring patients could obtain and understand their discharge medications.  In 2016, she took over as the supervisor for the Transitions of Care and Medication Reconciliation team and oversaw the successful implementation the MARQUIS 2 (Multi-center Medication Reconciliation Quality Improvement Study) best practices regarding gathering medication histories. She currently develops system-wide interventions to increase patient access to pharmacy services.
