Bio:
Dr. Jeremiah Duby is board certified in pharmacotherapy (BCPS) and critical care (BCCCP).  He is a clinical pharmacy specialist and residency program in critical care.  Dr. Duby primarily serves as the Surgical Critical Care service and periodically provides support for the Burns, Neurocritical Care, and Medical ICU services.  Patient care and bedside clinical challenges are the inspiration for his efforts in policy and protocol development, research, and teaching. 

Dr. Duby also serves as an Associate Clinical Professor for UCSF School of Pharmacy, UC Davis School of Medicine, and Touro University College of Pharmacy (TUCOP).  He precepts UCSF students and provides didactic lectures for TU students on acute care topics that include clinical pharmacokinetics, acute pain, sepsis/septic shock, acid-base disorders, and acute dysglycemia. 

Jeremiah graduated from from Washington State University (2003) and then a completed residencies in practice residency (PGY-1, 2004, William L. Fritz, MS, FASHP) and in critical care (PGY-2, 2005, Brian L. Erstad, Pharm.D., FCCM) at the University of Arizona Medical Center (UAMC).  His first job was an infectious diseases pharmacist for Kaiser Permanente (2005 – 2007); he helped pilot and establish a successful antimicrobial stewardship service with Stephen M. Parodi, MD, which later became as model of patient care for the region.

Tricia Parker, PharmD, BCPS offered Jeremiah a vision and a job in critical care.  The staff position started as split between ops and clinical responsibilities.  Over the course of a decade, the dream and collaboration grew to include brilliant colleagues, residency program, and clinical services.  The publications and awards are the product of teamwork and passion for patient care.
