Practice Philosophy:
I believe that as pharmacists, we fill an important role in ensuring that medication management plans are safe, effective, accessible, and patient centered. My goal is to work with our multidisciplinary teams to optimize care plans in a way that is individualized based on each patient’s unique needs, and to help patient’s feel comfortable and confident in managing their medications successfully.

Bio:
Dr. Shannan Takhar is a board-certified ambulatory care pharmacist at UC Davis Health. She received her PharmD from UCSF School of Pharmacy in 2018 where she held leadership roles with the Educational Policy Committee among various other positions and was inducted in the Rho Chi Honors Society. She went on to complete her PGY1 and PGY2 residencies in ambulatory care at UC Davis Health. In August 2020 at the completion of her PGY2 residency, she was recognized as Resident of the Year.

Following completion of her ambulatory care PGY2, she accepted a position with UCDH, focusing on pulmonology specialty pharmacy, including cystic fibrosis, interstitial lung disease, and a special emphasis on launching pulmonary hypertension specialty pharmacy services. In April 2021, Dr. Takhar accepted a new position supporting the ambulatory care solid organ transplant team and specialty pharmacy services. She is currently serving in both the solid organ transplant and pulmonary hypertension clinics in the interim. She contributes to the care of patients by seeing patients in clinic (or via telemedicine) during multidisciplinary visits with the care team providing comprehensive medication counseling, improving medication access and adherence, and making medication management interventions.

During her time as a staff pharmacist at UC Davis Health, Dr. Takhar has remained involved with the residency program by serving as a resident mentor and through precepting a multi-site resident research project. She loves to work with learners and is passionate about helping our students and residents grow into outstanding pharmacists.
