Health Care Transition

Health care transition is a process that prepares adolescents to take care of their health care needs when they reach adulthood.

What is Transition of Care?

During childhood, family and caregivers take the lead on making appointments, talking to providers and making health care decisions. However, they are also essential with providing transportation, refilling and administration of medications and learning how to use medical equipment, if necessary.

Once the adolescent becomes an adult, especially after they have transferred to adult care, they are expected to take full responsibility for all of their health care needs.

Preparing for Transition of Care

Transition from pediatric to adult care is a process that starts at the age of 12. It involves education, coaching and providing support until the pediatric patient can use the health care system independently as an adult.

The transition of care process will be customized to the meet the needs of the individual patient in a way that aligns with each family's values and culture. During the transition process the patient's needs will be addressed in an age and developmentally appropriate manner, with collaboration and support from the patient's family and caregivers.

Health Care Transition Topics

  • Educate the patient about their condition(s), complications and medications.
  • Practice independence by talking to providers, making appointments, filling prescriptions, etc.
  • Referral to special clinics to help with reproductive counseling, mental health issues and avoiding high risk behaviors, such as unsafe sex and sexually transmitted infections (STIs).
  • Explore future goals in life, including education and career.
  • Assistance with health insurance.
  • Find support in the community, eligibility for housing and acquiring social services, if needed.
