Preparing for Transition of Care

Transition from pediatric to adult care is a process that starts at the age of 12. It involves education, coaching and providing support until the pediatric patient can use the health care system independently as an adult.
The transition of care process will be customized to the meet the needs of the individual patient in a way that aligns with each family's values and culture. During the transition process the patient's needs will be addressed in an age and developmentally appropriate manner, with collaboration and support from the patient's family and caregivers.
Health Care Transition Topics
- Educate the patient about their condition(s), complications and medications.
- Practice independence by talking to providers, making appointments, filling prescriptions, etc.
- Referral to special clinics to help with reproductive counseling, mental health issues and avoiding high risk behaviors, such as unsafe sex and sexually transmitted infections (STIs).
- Explore future goals in life, including education and career.
- Assistance with health insurance.
- Find support in the community, eligibility for housing and acquiring social services, if needed.
