Data Collections
Collections of uniform data (clinical or other) about a population defined by disease, condition, event, or procedure. These are developed to support multiple ongoing, longitudinal efforts which may encompass quality of care, clinical effectiveness, research, or education.
Highlights
Cancer Center Data Informatics Integration Initiative (CCDI3)
The CCDI3 domain-specific unit (DSU) was established under the Data Provisioning Core (DPC) to develop local expertise and information resources to improve health outcomes of cancer patients and survivals, accelerate translational research capacities, and advance a learning health system at UC Davis Health. So far, the initiative is developing tools and operational dashboards and curating data linking phenotypes, life style factors, biomarkers, genotypes, health monitoring data, and other genomics data to optimize individual treatment plans and longitudinal evaluation and study.
Pulmonary Critical Care Medicine (PCCM)
The PCCM domain-specific (DSU) unit was the first to be piloted under the DPC model to curate patient outcomes and measures for critically ill inpatient adults. Metrics are available for hospital length of stay (LOS), hospital free days, ICU LOS, ICU free days, ICU bounce backs, ventilator
time, and ventilator-free days.
Sepsis
The sepsis project was initially implemented in 2011 to create a sepsis
patient cohort based on clinical criteria and then expanded in 2013 to
include Delivery System Reform Incentive Payment (DSRIP) reporting. In October 2015, Sepsis Core Measure reporting instituted a new cohort and bundle compliance metrics. The work of the Data Center of Excellence, Data Curation team virtually automated quality nursing evaluation of specific targeted cases mandated by Centers for Medicare and Medicaid Services. The sepsis algorithm is shared with all other University of California medical systems and provides the data for the Sepsis Quality & safety dashboard.
Business Objects Universes
SAP Business Objects supports universes which link data with business logic that allows for more complex analyses in a self-service environment without complex coding. The following universes have been developed to support several business functions.
Analytical Registries
Developed by the Data Curation Team, the success of these registries is made possible through strong partners with subject matter experts to retrieve, curate, and validate population cohorts and associated metrics.
Perioperative Blood Management - Designed to track and monitor blood use in elective surgeries since July 2014 to drive a more consistent standard of care related to red blood cell transfusion protocol for anesthesiologists. This data mart supports the Transfusion Quality & Safety dashboard.
CT Surgical Blood Management - Provides transfusion data for specific cardiothoracic (CT) procedures since May 2014 to facilitate data collection and population into the Society of Thoracic Surgeons National Database and Registry software program.
Diabetes – Expanded upon the Epic diabetes data mart by refining the cohort definition to include additional clinical criteria such as lab values, gestational diabetes, or diabetic complications diagnoses at any point since January 2003.
Dialysis – Derives adult and pediatric dialysis session and patient safety metrics since November 2012. The registry replaces a legacy system, freeing ~1.5 FTE from manual data entry.
Patient Centered Medical Home (PCMH) – Establishes a UC Davis Health definition of “medical home” patients; provides metrics on chronic disease management and preventive care. Supporting the Primary Care Network PCMH initiative since December 2012.
Pulmonary and Critical Care Medicine (PCCM)- The PCCM data mart contains all critically-ill adults admitted to the UC Davis Medical Center beginning in 2008, as well as nested sub-cohorts of critically ill patients and patients with pulmonary disease. The data mart contains a variety of metrics including those related to patient flow, patient phenotyping, and clinically-relevant patient outcomes.
Sepsis Core Measure – Measures sepsis cases and bundle compliance per CMS guidelines since October 2015. Utilized by the Sepsis Improvement Collaborative to review protocol effectiveness in reducing sepsis-related mortality and patient length of stay. Used for the Sepsis Quality & Safety dashboard.
Transfusion Medicine – Harmonizes blood product data with patient outcomes (Epic EHR and administrative data); includes all units dispensed since July 2011. Foundation for physician education efforts as part of Choosing Wisely campaign around blood ordering and utilization practices. Provides data for the Transfusion Quality & Safety dashboard.
Surgical Site Infection (SSI) Prevention- Enterprise data mart to assess adherence to preoperative antibiotic prophylaxis guidelines for the prevention of SSI. Surgical procedure categories include Obstetric & Gynecologic, Cardiothoracic & Vascular, Gastrointestinal, Head and Neck, Neurosurgical, Orthopedic, Urological, and Colorectal. The project was chartered by UC Davis Health Council of Chairs at direction of UC Davis Health Executive Leadership, David Lubarsky, M.D., M.B.A. and the SSI Committee members in coordination with the Quality & Safety team and Infectious Control
