‘Popular Science’ names Remote Patient Monitoring program a Top 50 Innovation of 2025

I’m thrilled to share that Popular Science has named UC Davis Health’s Remote Patient Monitoring program one of the 50 greatest innovations of 2025.

The Remote Patient Monitoring program helps patients manage hypertension at home and achieve real, measurable improvements. On average, participants reduced their blood pressure from 150/80 mmHg at enrollment to 125/74 mmHg—progress many continue to maintain after they graduate.

How the program works

  • Patients use connected blood pressure cuffs to securely share readings from home.
  • A multidisciplinary Population Health team—physician, pharmacist, nurse, and medical assistant—monitors data in real time and adjusts care plans.
  • The team provides group classes, one‑on‑one coaching and medication support.
  • The program lasts six months, with the option to extend up to one year to reach personal goals.
