22q Healthy Minds Clinic
The 22q Healthy Minds Clinic has a team of experts that includes a developmental pediatrician and a licensed clinical psychologist. They offer clinical assessments specifically for individuals with 22q11.2 deletion (VCFS/DiGeorge) syndrome. Currently, these assessments are offered on the fourth Monday of every month.
The following services are offered to children ages 14 and younger:
- Developmental or Neuropsychological Examinations (ages 14 and younger)
- Medical Consultation
- Consultation/Management of Medications
Families who would like their children seen in our clinic should:
- Get in touch with their insurance company to confirm that their medical and behavioral health benefits cover the services we offer, and ensure that they have the necessary authorization.
- Schedule an appointment with their child's primary care physician (PCP) to discuss their concerns and explain why they want their child to be examined at our clinic.
The primary care physician has some important responsibilities:
- Provide a written report that includes the child's medical history, physical examination findings, and the reasons for the referral.
- Request authorization from the patient's insurance company for specific Current Procedural Terminology (CPT) codes that are necessary for the consultation/evaluation. These codes are: 96112, 96113, 96116, 96121, 99205, 95354, 99367, 96132, 96133, 96136, 96137, and 99366. The PCP can review our sample medical justification letter in PDF format if needed.
- Fill out the MIND Institute Clinic Referral sheet (available in PDF format) and fax it to 916-703-0350.
- Call the UC Davis Physician Referral Center at 800-482-3284.
After completing these steps, parents can contact June LaMarr, our New Patient Referral Coordinator, at 916-703-0440 to start the referral process.
- Patient's current and past medical and developmental history, along with the last PCP appointment documenting a physical examination.
- School/Agency records if applicable, including current 504 plan, Individual Education Plan (IEP), most recent psychoeducational or eligibility evaluation reports, Individual Family Service Plan (IFSP) or Individual Program Plan (IPP).
- Copies of previous diagnostic or psychiatric evaluation reports, including Mental/Behavioral Health, Speech/Language, Occupational Therapy, etc.
- Completion of a patient intake questionnaire and other behavioral questionnaires prior to the appointment.
- Receives referral with PCP’s History and Physical Examination report.
- Gathers supporting records.
- Triages to appropriate specialty services.
- Obtains insurer’s authorization for specialty services.
- Appointment scheduled.

Kathleen Angkustsiri, M.D.
Dr. Kathleen Angkustsiri is a developmental-behavioral pediatrician with interests in neurodevelopmental disabilities including autism, chromosome 22q11.2 deletion syndrome (22q11.2DS), and fragile X syndrome. Dr. Angkustsiri is involved in research on dysmorphology for children with autism, behavioral characteristics of children with 22q11.2DS, clinical trials for autism, and fragile X syndrome.

Janice Enriquez, Ph.D.
Janice Enriquez, Ph.D., is a licensed clinical psychologist within the Developmental and Behavioral Pediatrics Section at the MIND Institute who conducts developmental evaluations with infants and children to identify neurodevelopmental concerns related to intellectual and learning disabilities, autism spectrum disorder, ADHD, and related mental health concerns.
