  • Services provided must be medically necessary and rendered by qualified health care professionals in accordance with the scope of practice and federal and state laws.
  • Assignment of CPT, HCPCS, and ICD-10 codes is based on the medical record documentation and payer specific guidelines.
  • Medical record documentation must meet the requirements outlined in UC Davis Health Policies and Procedures (P&P) 2307 and as applicable in P&P’s 1930 and 1905.
  • With a limited exception, teaching physician services may be billed to Medicare only when the documentation supports that the teaching physician provided a direct service to the patient.
