Ensure safe, effective, and optimized medication regimens for patients throughout their continuum of care. Advocate for patients by working to resolve cost barriers to ensure affordable medications. Inspire learners to grow into independent practitioners by providing support, role modeling, and encouraging them to incorporate primary literature and guidelines into their practice.

Bio:
Dr. Muthusi-Asiago graduated from the University of Illinois at Chicago, College of Pharmacy in 2011. She then completed a PGY1 Acute Care Pharmacy Practice Residency at Mercy Medical Center-North Iowa in 2012. Dr. Muthusi-Asiago is board certified in pharmacotherapy.

Upon completion of residency training, Dr. Muthusi-Asiago accepted a position with Skiff Medical Center in Iowa as a part-time inpatient pharmacist and then transitioned to the Iowa Veterans Home as a staff pharmacist. She later accepted a position as Clinical Inpatient Pharmacist with the Department of Veterans Affairs (Des Moines, IA) where she spent 7 years. At the VA, she was actively involved with the expansion of inpatient clinical pharmacy services including interdisciplinary rounds, anticoagulation management, total parenteral nutrition management, and antimicrobial stewardship.

Dr. Muthusi-Asiago serves in her current role as a Population Health & Transitions of Care pharmacist at UC Davis Health. In this role, she works with patients admitted to UCDH affiliated skilled nursing facilities (SNFs) and the SNF providers to identify and resolve medication related problems throughout the patients’ continuum of care from admission to discharge. She is also involved with population health related initiatives such as CMS STARs measures and multiple Quality Improvement Pool Program (QIP) initiatives.

Following completion of her PGY-1, Dr. Tina Ho accepted a position with UC Davis Health as a Hepatology Float Clinical Pharmacist in the Specialty Pharmacy Clinics focusing on hepatitis B and C management.
