Practice Philosophy:
My goal is to provide comprehensive, compassionate, guideline-directed care that enables me to build trusting, empowering relationships with my patients. Through a shared decision-making model, I work with my patients to address their health using both lifestyle modifications and medication management. I am passionate about serving the underserved and hope to impart this passion on my learners.

Bio:
Dr. Noelle Nelson is an ambulatory care clinical pharmacist at One Community Health, a Federally Qualified Health Center in Sacramento. Dr. Nelson earned her Bachelor of Science in Biological Sciences with a minor in Psychology from UC Davis and subsequently went on to matriculate at Campbell University College of Pharmacy and Health Sciences in North Carolina. Dr. Nelson graduated Summa Cum Laude from Campbell University with dual Doctor of Pharmacy and Master of Science in Public Health degrees. After relocating back to her home state of California, Dr. Nelson went on to complete an HIV/Ambulatory Care PGY1 Residency and a PGY2 Ambulatory Care Residency at UC Davis Health.

Following completion of her PGY2, Dr. Nelson accepted a clinical pharmacist position with UC Davis Health and One Community Health, focusing in primary care, HIV, and hepatitis C management.
