Victor Castillo Garcia, Jr. provided extraordinary care for his patient. This is a patient that had a 50-day length of stay. This was a very difficult to discharge patient. The challenge was that the patient was requiring a short-term skilled nursing facility (SNF) rehab that would be transitioned into long-term custodial care in a SNF. Over 365 facilities were referred to, including Southern California SNFs. We only had a SNF in Los Angeles (LA) that was willing to accept the patient. The family pleaded with Victor to keep the patient in the Sacramento area because it would have been a hardship for the family to visit the patient in LA and may attribute to a faster decline of the patient if family is not able to visit him. Victor was requested by Jane and I to continue to task the placement of the patient with Keya and Karen, but he was determined in finding a solution that would meet the family’s wishes. Victor called all the SNFs that this patient was referred to in the Sacramento region rather than depending on continuing to send referrals through All Scripts. With Victor’s determination and efforts to keep the patient in the region where his family can continue to visit him, he was successful in getting a Sacramento SNF, Sherwood Healthcare, to accept the patient. Victor’s benevolence to serve our patients and their families in the best way possible is very much appreciated.
