Professor of Internal Medicine and Director of the Center for Reducing Health Disparities (CRHD)

Sergio Aguilar-Gaxiola, M.D., Ph.D. is Professor of Clinical Internal Medicine, School of Medicine, University of California, Davis. He is the Founding Director of the Center for Reducing Health Disparities at UC Davis Health and the Director of the Community Engagement Program of the UC Davis Clinical Translational Science Center (CTSC). 

Director of Education, Center for Reducing Health Disparities

Dr. Hendry Ton is the Associate Vice Chancellor for the Office for Health Equity, Diversity and Inclusion, Associate Dean for Faculty Development and Diversity, and a Clinical Professor of Psychiatry and Behavioral Science. 

Assistant Director of Education, Center for Reducing Health Disparities

Dr. Chadha is an Associate Professor and Clinical Psychiatrist, Medical Student Site Director for Adult Psychiatry Support Services, the Director of Diversity Education with the Office of Faculty Development & Diversity, and teaches Co-IOR Doctoring 3. 

Chief Administrative Officer

Andrea Nuñez serves as the Center for Reducing Health Disparities' Chief Administrative Officer and joined the Center in 2017.

Projects and Personnel Manager

Maria Alaniz is the Projects and Personnel Manager for the Center for Reducing Health Disparities. She joined the Center in 2018.

Project Manager

Gabriela Alaniz is the Project Manager for the Digital Health Equity project in the UC Davis Center for Reducing Health Disparities. She joined the Center in 2021.

Student Administrative Assistant

Yanele Ledesma is a Student Administrative Assistant working primarily with social media. She joined the Center for Reducing Health Disparities in 2023.

Data Analyst

Emaan Ahmed is a Data Analyst for the Licensed Physicians from Mexico Pilot Program (LPMPP). She joined the Center for Reducing Health Disparities (CRHD) in 2024.

Digital Health Navigator

Taliyah Borromeo, currently serving as a Junior Specialist in the Department of Internal Medicine for UC Davis Health, also holds the role of Digital Health Navigator for the Digital Health Equity Project at the Center for Reducing Health Disparities.

Project Manager

Katherine Ferry (she/her/ella) is the Project Manager for the Accountability, Coordination and Telehealth in the Valley to Achieve Transformation and Equity (ACTIVATE) project and the California Reducing Disparities Project (CRDP) Latino Technical Assistance Provider grant. She joined the Center for Reducing Health Disparities in 2022.

Clinical Research Coordinator

José Florez is currently the lead Clinical Research Coordinator for the current Digital Health Equity project for the Center for Reducing Health Disparities. He joined the Center in 2022.

Junior Project Coordinator

Jacob Fraga is the Junior Project Coordinator for the Together For Wellness | Juntos por Nuestro Bienestar project. They joined the Center for Reducing Health Disparities in 2023.

Data Analyst

Yury García became part of the Center for Reducing Health Disparities in 2023. She supports the All of Us project as a Data Analyst.

Digital Health Navigator

Monica Grajeda Hatch is the Digital Health Navigator and Administrative Coordinator for the Digital Health Equity Project in the UC Davis Center for Reducing Health Disparities. She joined the Center in 2022.

Special Projects Coordinator

Joseph Hernandez serves as the Special Projects Coordinator for the California Reducing Disparities Project (CRDP) and the Community Engagement Alliance (CEAL) STOP COVID-19 projects within the Center for Reducing Health Disparities. He joined the Center in 2022.

Elliott Housman-Turrubiate is the Project Manager for the Together for Wellness | Juntos por Nuestro Bienestar project. Elliott joined the Center for Reducing Health Disparities in 2022. 

Student Assistant

Natanya Kharat is a Student Assistant for the Together for Wellness | Juntos por Nuestro Bienestar project, and joined the Center for Reducing Health Disparities in 2023.

Digital Health Navigator

José Lastra is the Digital Health Navigator and Outreach Coordinator for the Digital Health Equity project in the UC Davis Center for Reducing Health Disparities. He joined the Center in 2022.

Senior Project Manager

Jill López-Rabin is the Senior Project Manager for the Interdisciplinary Collaboration and Cultural Transformation Model Learning Collaborative (ICCTM LC) and the Licensed Physicians from Mexico Pilot Program (LPMPP) projects. She joined the University of California at Davis’ Center for Reducing Health Disparities in 2018.

Project Coordinator

Krystal Mora Anaya is the Project Coordinator for the Interdisciplinary Collaboration and Cultural Transformation Model Learning Collaborative (ICCTM LC). Most recently she started project coordination for the Program to Alleviate National Disparities in Ethnic and Minority Immunizations in the Community (PANDEMIC). She joined the Center in 2022.

Karla Ornelas Hernandez is Project Coordinator for the Accountability, Coordination, and Telehealth in the Valley to Achieve Transformation and Equity (ACTIVATE) project. Karla joined the UC Davis Center for Reducing Health Disparities in 2021.

Angélica María Pérez is the Project Manager for the Digital Health Equity Project in the UC Davis Center for Reducing Health Disparities. She joined the University of California at Davis’ Center for Reducing Health Disparities in 2024.

Community Engagement Program Manager 

Vanessa Trujillo is the Community Engagement Program Manager for the Clinical and Translational Science Center and Center for Reducing Health Disparities at the University of California, Davis and joined in this role in 2021.

Kristen Weeks-Norton is the Project Manager for the California-wide, Community Engagement Alliance (CEAL) STOP COVID-19 project. She joined the Center for Reducing Health Disparities at University of California, Davis in 2022.

Heather Diaz, Dr.PH, M.P.H. is an Associate Professor at Sacramento State University in Health Sciences, assigned to the Community Health Education curriculum.
