• Although the BRSR attempts to avoid supplying specimens contaminated with highly infectious agents (such as hepatitis, HTLV-III, etc.), all specimens should be handled as if potentially infectious. The BRSR accepts no responsibility for any injury (including death), damage or loss that may arise either directly or indirectly from their use

• The investigator assumes all risks and responsibility in connection with the receipt, handling, storage and use of specimens

• The investigator receiving these specimens also assumes full responsibility for informing and training all personnel in the dangers and procedures for safe handling of these and other human specimens

• Anyone who obtains specimens from the BRSR should have adequate training in the use of bloodborne pathogens, as outlined by the Occupational Safety and Health Administration. For more information, visit the bloodborne pathogens webpage at UC Davis Safety Services

• The investigator must agree to indemnify and hold harmless the UC Davis Comprehensive Cancer Center and UC Davis from any claims, costs, damages, or expenses resulting from any injury (including death), damage or loss that may arise from the use of specimens provided by the BRSR

Most tissue procurement is performed by BRSR staff in the Frozen Section Room of the Department of Pathology at UC Davis Medical Center, located just outside the operating rooms. The location is ideal for the rapid retrieval of tissue after surgical resection. Following evaluation by the surgical pathologist, BRSR staff transport the specimens to the BRSR office/laboratory located one floor down from the frozen section room, where the specimens are immediately processed and either flash frozen/frozen or dispersed fresh (in RPMI media) to researchers.

• Patient identity is confidential and all specimens will be coded. The key to the codes will always remain with the BRSR. No protected health information will be released to researchers

• The BRSR does not supply samples to researchers solely for distribution to third parties; those researchers should apply to the BRSR directly

• The recipient agrees to acknowledge the contributions of the UC Davis Comprehensive Cancer Center in all publications resulting from the use of specimens obtained from the BRSR. Recommended wording to use in the methods or acknowledgement sections is as follows: "Specimens were provided by the UC Davis Pathology Biorepository which is jointly funded by the UC Davis Comprehensive Cancer Center Support Grant (CCSG) awarded by the National Cancer Institute (NCI P30CA093373) and the UC Davis Department of Pathology and Laboratory Medicine."
