Residency Verification of Training Process

The Department of Internal Medicine provides an official verification for completion of training using the standard verification letter. Training verification to current trainees or graduates within one calendar year of their graduation date are provided at no-cost.

Verification Fees

  • $50.00 - Standard UC Davis Internal Medicine verification form
  • $75.00 - Non-standard verification form, or your own institutional form
  • $100.00 - Custom verification letter, request for university seal or notarization

How to Request A Training Verification

  • Provide your own institutional verification form and release form (you may also request to use the Standard UC Davis Internal Medicine verification form)
  • The Program Coordinator will send you an invoice for the request
  • Send payment confirmation to imres@ucdavis.edu
  • Once payment is received, you will receive the verification letter within 5-7 business days by FAX or e-mail (please note the Program Coordinator is in the office twice a week)
