Frequently Asked Questions: Medical Education Documentation needed to obtain a Postgraduate Training License in California

Question:  How can I request the Medical Education Documentation needed for the California Medical Board Postgraduate Training License (PTL) Application. 
Answer:  You may request the completed Certificate of Medical Education (Form MED) and Certified Copy of Medical School Diploma from the Registrar’s Office.  Official Transcripts must be requested through the Main Campus Registrar.  Please see each section below for specific information. 

Question:  When can I request the Medical Education Documents (Form MED), Certified Diploma, and Transcripts with conferral date
Answer: These documents cannot be released until after your degree has officially been conferred.  The degree conferral date is typically the second Thursday in June of each Academic Year.

Question:  How can I request the registrar’s office complete the Certificate of Medical Education (Form MED)?
Answer:  Please attach the Certificate of Medical Education with the Applicant Information completed along with the completed Authorization of Release and send to hs-studentrecords@ucdavis.edu. Please include your application number and/or file number in your request so we can locate your record. The Registrar’s Office will upload your completed Certificate of Medical Education to the DOCS Portal.

Question:  How can I request a copy of my certified diploma for my PTL? 
Answer:  Please email your request and attach the completed Authorization of Release and send to hs-studentrecords@ucdavis.edu.  Please include your application number and/or file number in your request so we can locate your record.  The Registrar’s Office will upload a copy of your certified diploma to the DOCS Portal.

Question:  How do I request my Official Transcript for my PTL? 
Answer: Official transcripts are processed through the main campus Registrar’s office using this link: https://registrar.ucdavis.edu/records/transcripts. Please follow the instructions listed on the California Medical Board Website for electronic transcripts. The Board accepts electronic transcripts from Parchment (which is the vendor used by UC Davis).  Please designate the Medical Board of California as the recipient and submit transcripts to mbclicensingelectronicnotifications@mbc.ca.gov.  The Registrar’s Office does not upload your transcript to the DOCS Portal.  You must follow the instructions above. 

Question:  What if I am seeking licensure in another state? 
Answer:  If you are seeking licensure in another state, please be aware that each state has their own process you will need to follow.  Please compete the Authorization for Release and submit it via email to hs-studentrecords@ucdavis.edu along with any forms that need to be completed by the Registrar’s Office. Please note: Official transcripts are processed through the main campus Registrar’s office using this link: https://registrar.ucdavis.edu/records/transcripts.

All services of the records office are free of charge. Please allow up to 5 business days to process your request. 

All requests must be accompanied by an Authorization for Release (PDF) .
