- M.D. Degree Verification
- Certification of Diploma
- Completion of Licensure Forms
- Copies of Dean's Letter
All services of the records office are free of charge. Please allow up to 5 business days to process your request.
All requests must be accompanied by an Authorization for Release (PDF) .
