Question: How can I request the Medical Education Documentation needed for the California Medical Board Postgraduate Training License (PTL) Application.
Answer: You may request the completed Certificate of Medical Education (Form MED) and Certified Copy of Medical School Diploma from the Registrar’s Office. Official Transcripts must be requested through the Main Campus Registrar. Please see each section below for specific information.
Question: How can I request the registrar’s office complete the Certificate of Medical Education (Form MED)?
Answer: Please attach the Certificate of Medical Education with the Applicant Information completed along with the completed Authorization of Release and send to hs-studentrecords@ucdavis.edu. Please include your application number and/or file number in your request so we can locate your record. The Registrar’s Office will upload your completed Certificate of Medical Education to the DOCS Portal.
