How to apply

Applications for the 2025-2026 cohort of the Primary Care APP Fellowship are open.

The two-part application process includes:

  1. Completion of an online application at the UC Davis Recruit website
  2. A follow-up Supplemental Questionnaire which is sent to all applicants who meet the basic, minimum qualifications

Applications are not accepted after the final deadline. Invited finalists participate in a mandatory interview conducted via Zoom in March.

How to submit an application

Step 1 —Complete an online application at UC Davis Recruit. Note: the system automatically saves your progress as you work through the application.

Required documents include: 

  • Resume or Curriculum Vitae (C.V.): must include all education, leadership positions, community service experience, research activities, quality improvement projects, teaching experience, professional certifications and at least five years of work experience. A C.V. is an opportunity for you to tell us the story of your professional life. Applicants who have not completed their master’s or DNP program prior to submitting their application must indicate an expected graduation date in their C.V.
  • Contact information for two references who can speak to your qualifications and capabilities as a nurse practitioner: one must be a supervisor or someone who supervised you in your clinical practice. This may be a preceptor if the experience was in primary care and lasted at least 160 hours. Your two references will be contacted if you are offered an interview. 
  • Response to three essay questions (see information below)
  • Statement of contributions to diversity, equity and inclusion (see information below)
  • Graduate program transcript: graduates should submit a final transcript (official or unofficial). Current students should submit a transcript that shows up to and including their most recently completed semester or quarter. 
  • Explanation of gaps in work or school that exceed three months

Please note, additional materials outside these required elements are not considered.

Step 2 — Look for an email referencing a required Supplemental Questionnaire that is sent to you within 3 to 5 business days after the application period closes and it is deemed that you meet minimum qualifications. You must complete this questionnaire within three business days. 

Step 3 —Develop your plan to obtain all required documents that must be submitted as outlined on the Credentialing Documents Checklist PDFs below that applies to you.

For applicants who completed their graduate program between April 1, 2024, and Dec. 31, 2024: 

For applicants who completed or expect to complete their graduate program between Jan. 1 and June 15, 2025: 

Please answer the following three questions: For questions 1 and 2 combined, there is a 1-page limit.  Please include your responses to questions 1 and 2 on a single page. For question 3, there is a 2-page limit.

Question 1 (must fit on one page combined with Question 2)

Describe your career direction and goals as they pertain to your role as an advanced practice provider.  Then, describe how you have prepared yourself for this chosen career. 

Question 2 (must fit on one page combined with Question 1)

Describe your motivation to join UC Davis’ Primary Care APP Fellowship program.  How will your background and experiences help you provide primary care to members of the fellowship's priority communities?  You may include a discussion of your background or challenges in any of the following areas: familial, cultural, economic, social, residency/citizenship, academic, significant life or other experiences. 

Question 3 (2-page limit)

Select only THREE of the following six areas that have been most significant to your professional development thus far:  a) leadership, b) teaching, c) scholarly/creative/research, d) quality improvement, e) public, community or other service, and f) team care or interprofessional teamwork. Your chosen three topics can be the result of your academic, work or lived experiences.  For each of the three areas selected, describe your experience and how you anticipate it will impact your role as an independent primary care provider. Use headers to clearly identify your topic areas.

Applicants are strongly encouraged to prepare the documents listed on the checklist that applies to their specific situation (determined by graduation date and location of educational program). The items are required at various points before the fellowship begins. Please refer to the Credentialing Document Checklists (see links to PDFs above).

The Primary Care APP Fellowship position is a full-time, 12-month faculty appointment without possibility of extension. Primary care APP fellows receive a salary* and full benefits including paid vacation and holidays. 

Primary care APP fellows should expect to work primarily day and early evening hours, Monday through Friday; however, there may also be required skills training, conferences, or clinic days that take place on weekends. 

Successful fellows complete all 12 months of the fellowship, build a portfolio suitable to seek employment, receive a letter of recommendation for employers and are considered for open positions at UC Davis Health or potentially at FQHC partner sites or program affiliated organizations.  

Fellows cannot pursue employment opportunities until the last three months of the program and must complete the full 12-month fellowship.
